Welcome to my practice! I'm Anna Jacobi, a board-certified nurse practitioner with over 20 years of healthcare experience. My mission is to help individuals optimize their health and prevent or reverse chronic diseases. In a world filled with online options, I strive to create a personalized setting where patients feel supported on their health journey.

My Story

My medical journey began in 2004 when I enlisted in the Iowa Army National Guard as a combat medic. During my two deployments to Iraq, I was inspired by the dedication and skillset of the nurses caring for the sick and severely injured. This experience motivated me to return home and embark on my own nursing career.

 

I’ve spent 10 years working as a registered nurse working in hospital settings and another six years as a nurse practitioner in the primary care setting.  Many individuals find themselves caught in a cycle of chronic disease, often requiring multiple medications to manage their conditions. My goal is to break this cycle by empowering patients to take charge of their health, focusing on effective weight loss strategies and holistic wellness. Together, we can work towards achieving a healthier, more vibrant life.

I grew up in the Midwest and did all of my nursing training in that area.  Seven years ago, I moved to Oregon to start my career as a nurse practitioner.  My family and I live on an acreage near Scio, where we share our lives with a variety of animals including dogs cats, ducks, goats, and chickens. We love taking care of our furry and feathery friends while tackling house projects together.

With extensive education and years of hands‑on experience, she helps patients build strength, confidence, and sustainable habits through a balanced, approachable style. Megan’s holistic mindset and supportive teaching align perfectly with our mission to guide patients toward healthier, more empowered lives.

My Story

My journey with health and wellness started over 15 years ago when I started teaching
water aerobics at the local Y in my hometown. I fell in love with teaching and continued
my education in the wellness field when I went to college at the University of Iowa. I
graduated with a degree in Health Promotion and certifications as a personal trainer and
group fitness instructor. Out of college, I began teaching health and physical activity
courses at the University of Iowa and have been there for the last 10 years. During this
time, I completed my masters in Health and Wellness Management, training to become
a Registered Yoga Teacher, and became a Certified Nutrition Coach. My education has
allowed me to teach a variety of individuals about the importance and impact of
exercise, diet, sleep, and stress management.
Throughout my career, I’ve not only gained insight from my education, but my own
personal struggles with health. I have experienced autoimmune related issues that were
hard to fix and find answers for in conventional medicine. This pushed me to look for
answers within my own knowledge base of wellness. I’ve found many answers along
the way, which I now want to share with others. My goal as a wellness coach is to listen,
support, and tailor the approach we take to your goals, life, and resources you have
available. I’m excited to get to know you and take the journey to good health together.
Outside of work, I spend most of my time with my two little boys and our dog. Our family
lives on an acreage in Iowa where we spend as much time outdoors as we can. I enjoy
gardening with my boys in the Summer, going on walks with my husband, and spending
time with our family.
